    There is no any matching name in both sheets. Are you sure you have attached right Index or name list?

    I just did a quick scan of your excel sheet and there a few names in "Names to Complete" tab which are not there in "Index". DO we leave them blank or look for them somewhere else?

    Based on this I can send a proposal.
