Estimated project duration: 1 - 2 weeks
We require a means of capturing data from 3 different hard copy forms.
It is anticipated that the information from the hardcopy forms will be manually entered elecronically (fillable form in excel, etc) and that this data will be captured and presented on a database. The data should also be automatically be summarised in graphs/percentages as each form is inputted.
The tree forms are attached. The first form () will be complicated to create a form and database for and will require some creativity. The other two forms () are more simple.
Here is the background:
Form 1 (SENSORY LOSS STUDY): We are conducting a study/survery on our medical department. The first form is related to a study which is designed to assess how much nerve damage a patient feels after surgery over a period of 2 years. The same patient will fill the same form over a number of time periods during this 2 years (i.e. before surgery, after surgery, 1 month later, 6 month later, etc). As each form is inputted electronically, i want the data to be A) captured and B) summarised. After some time we will have hundreds of forms inputted and we will require the setup to show us a summary of the data at any given time.
This is complicated becasue there are multiple zones on the face to consider. We may need to discuss this further.
Form 2 and 3 (PLACEMENT and FAILURE) - These two forms are much simpler and relate to a study regarding the placement of implants. One form records data for implant placement. The other form records data for implant failure. Similar to form 1, we will input the data manually and we require for this data to be captured and summarised.
