Build Database

Requirement is for a database which can be used by multiple users to add, store, update client contact and status information. Users must be able to add notes following conversations with clients. The database needs to automatically store multiple notes along with the date and user who made the notes.
Statuses will be used to indicate what "life-cycle" stage the clients are at. We would like this status to determine what other fields are available on the record to be updated.
We are currently building a website and we want the database to be used via this website.
Some reporting will be required from the database.
Full spec will be provided before work commences.
