Access 2010 database of employee details for small business

We require an Access 2010 database to:
•store personal details of all of our permanent staff and freelancers (currently 30-50 individuals but likely to increase in future)
•search for employees with certain skills/qualifications from multiple fields.

The information will be entered into the database from paper forms (electronic copy attached), and the desired design of the data entry form on the database will reflect the design of this paper form for ease of data entry. Some fields will require multiple values to be entered, such as ‘Skills’ and ‘Work experience’. We require this database to be completed as soon as possible within 3 weeks.
