Using Microsoft Forms to Create a Document

Hi, I would like to have a form created using microsoft forms which will enable me to send the blank form for data collection to enable me to keep track of project management information needed.
Is this possible using Microsoft or should we move to a subscription based software.
Some of the questions can be multi choice, some can be mandatory free form
Some can be numbers / dates or text.
Need help with the initial design, layout etc and recommendations
