Survey report writing

I need someone to create survey analysis reports based on survey data. Reports should contain charts (where relevant and helpful) as well as statistics and summaries of the data results. Most surveys have roughly 20 respondents and around 20-30 questions. Each report can be anywhere from 1 to 5 pages depending on complexity. Payment: 100-500 USD per report.
Person will receive 1 survey to start with a more will come if the quality is good.

Typical structure of reports:
- summary of results in bullet points (executive summary)
- analysis of all questions excluding those which do not yield significant results.
- analysis can be in both chart or text form.

Requirements:
- Excellent skills in data/design (eg. creating charts) not just boring excel charts, but nicely designed ones!
- High level in English. Only high level or native English speakers.
- Provide examples of your work

Background
I am a consultancy that works within the domain name industry for top level domain operators. Surveys are on the topic of registry business, policy and technical aspects of domain name registrations.
