Help building a survey monkey survey - and analysing the results

I need someone to create a survey in survey monkey, with around 14 questions, which we will write.
The survey is around cultural values (within organisations)

I need someone to analyse the questions and see if anything needs to change to help us achieve the right outcome, then advise us on how to change them.

As well as building the survey we also need someone who understands:
1. How to build it so we can generate the insight we're after
2. How to generate insight that meaningful from the survey responses
3. Help us interpret meaning
