Wordpress - New page with 100 Question Form on.

Please see attached a Questionnaire with questions on that need putting onto our wordpress site.

There are questions that require tick box answers and then some that require text boxes. Not all boxes are mandatory.

Once the customer answers this they will have a 'Send button' where the answers are sent to an email address in an excel format or similar.

The Styling and Format will be to be discussed.
