Simple website with DVLA form integration

I am looking for a simple website with a form that integrates dvla vehicle registration look up either via DVLA api or a paid plugin (advice required ) -

I require a car registration form to be added to the website which will allow people to enter their car registration and it will take them to the next page / section / icon to confirm the vehicle info provided is correct (info by dvla) for example see https://sellcar.co.uk/ (if possible to create page that looks similair with pic of car and the form embedded in there, to resemble number plate) - and then they will fill the rest of the form and it will then be submitted to an email address.

It must be similar to the above website.

There is a basic wordpress site is already there .. just needs to be one page with links for terms conditions / gdpr / privacy policy Needs to be done asap and really simple. No products to list.
