Quote Calculator for Wordpress Website

I am finishing off a site which can be seen here: http://quickshipfulfilment.co.uk/

An online pricing calculator is required to provide users with a rough quote as to how much the fulfilment will cost them, similar to this: http://www.corefulfilment.com/pricing/

However, although the sliders are great, the ability to manually enter the quantities is also required in the boxes.

Also, an email/name must be provided before any quote is shown/emailed.

I can provide access to the WP project and there are many plugins out there that do similar so it doesn't matter if you adapt these.

I am a developer, but as you will see from other jobs I have posted on PPH, I have lots of little bits and pieces, mostly in PHP that need doing and I would love to find someone to work with ongoing.
