Landing Page optimization for PPC campaign

We have a website currently built in WordPress:

I am looking to optimize the design and content of 1 page for a new PPC campaign on Google Adwords to maximize conversion rate.

Target keyword will be: "embroidery" and other 2-3 word variations on this.

Current landing page is: http://www.indigoclothing.com/embroidery/

This page was originally optimized for SEO but from a CX perspective I think it performs poorly.

The conversion goal is for people to contact us for a quote via this page:
http://www.indigoclothing.com/quote/

Notably our page does not include garments which could also be a mistake so happy to include 3-4 featured products such as polo shirts or sweatshirts with a promotion on to make the page more compelling.

It is also an option to have a version of our contact form inline.

How do you provide the design - in PSD?

Are you also able to actually implement via Wordpress/HTML/CSS?

Please provide time estimate and example of previous projects.

If project goes well will expand to optimize other pages for new PPC campaigns.
