I want to customise the product page into a "survey" which allows me to sign up leads

I run a business which offers a personalised service delivered to the customer every month. Right now my sign up flow is simple like the WooCommerce flow, and I would like to change that. The user flow is as follows:

*Customer Presses Sign Up* - Form unfurls or new page opens

- Asks if this is a gift
- Asks the customers preferences (these will be different for each product we sell)
- Asks the customer how long they would like to receive boxes for (this will be billing cycles)
- Asks the customer when they would like the box to ship (eg May, June, July) auto-updating.
- Finally generates a price, and if the customer presses continue sends the customer to checkout.
