I need to design/style already existing Donate pages

Hello,

We're a small non-profit and need styling for our donate pages. There's an initial landing page for donation, where the visitors can select one of three currencies (http://www.wijngaardsinstitute.com/make-a-donation/), and then we have 3 identical donation pages, one for each currency (e.g. this is the one for USD: https://www.wijngaardsinstitute.com/donate-en-us/).

As I said, their styling needs improving. I'm attaching a document (.docx) I'm attaching a document where I describe what we're thinking of having - not exactly a wireframe, but enough to get an idea of the layout.

Any question please ask. Suggestions with regard to our proposal in the document attached are also welcome.

Please note that the donation form has been built using Gravity Forms, and both Stripe as well as PayPal.
