Custom WordPress submit form ported from one website to another and fields changed

Description of requirements/features: I have a custom form developed for WordPress, hardcoded into the template. When submitted the form does a number of things:

1. Creates a custom post type "quotations" with all of the form's fields populated into the post type's custom fields for website owner review, reference and editing
2. Branded email sent to a given email address with details of the submission
3. Creates a branded PDF document featuring all submission details

I need this form moved to another website and re-purposed, with different fields and questions. All other functionality must be kept, of course reflecting the updated form fields as well.

I can provide FTP and WP access to both original and target websites.
