Custom LearnDash registration form

We are using LearnDash 3 with Uncanny LearnDash Toolkit (and Formidable Forms in a Wordpress / DIVI environment)

Requirement is to replace/create a Custom registration form:

First Name - required
Last Name - required
Email - required AND needs to only allow/deny specific domains e.g. allow:mycompany.com, myothercompany.com | deny:gmail.com, outlook.com. Display error on deny.
Location - dropdown of LearnDash groups

You must be experienced with Learndash and theie registration process to bid for this - please describe previos exerience or links to sites.
