A website with e-commerce

Hello, I need a simple but well-designed website made. It should be something similar in scope to this website: www.paycheckprotectionplanner.com. It needs to have the following elements:

(domain is hosted at GoDaddy.com)

Navigation (pages)

Home
One page of text and photos

Sign Up
Form automatically pops up when someone comes to page, and also it is the second link in the navigation. The Sign Up form should have:
First Name, Last Name, Title, Organization, Street Address (use google map), City, State, Zip Code, Phone, Email, box to check that says, “Keep me updated about more Covid 19 Resources.” (make that box already checked by default).

About
One page of text and photos, and also a youtube or vimeo embedded video

Resources
A page of items for people to download:
• Covid19 Action Planner (PDF download, with photo)
• Corona Care Guide (PDF download, with photo)
NOTE: The first time they click to download an item, they must fill out the Sign Up Form.
NOTE: I want to be able to easily upload more items on this page, through the admin. When I upload them, I want to put the name of the item, a photo of the item, and the actual file for people to download

Order Supplies
Page of items to order. Should be a simple e-commerce page which I can easily add products to on my own (through the admin). It needs to have this info for each product:
• Product Photo (should allow up to 4 photos per product)
• Product Name
• Description
• Price
• Quantity
• Total
• Shopping Cart that shows all items selected
• Tax (for California orders)
• Shipping options
• Payment method (credit card, paypal, venmo)
• Check out
• Email confirmation sent to buyer
