LegBank is searching for a way to create a prototype that allows to collect data by deploying digital forms, through a very simple user interface on a smartphone and/or desktop computer, both offline and online.
How?
We would like to convert a set of existing forms (approx. 10 forms) into a digital form accessible via a mobile app. Those forms should be able to be used by a small group of test users and deployed throughout their process at different moments in time. Meaning, from patient intake and patient assessment, moving forward to prosthetics manufacturing, finishing with prosthetics delivery and patient satisfaction assessment. Once filled in, those forms should be able to be stored and easily accessible for whenever needed by the test users themselves (not us).
Looking into existing applications, we found Formotus; a solution that allows for turning forms into mobile apps. Formotus works with Microsoft's InfoPath for form creation and Microsoft's SharePoint (or other standards-based XML Web services - SOAP/REST) for creating workflows and as backend.
LegBank is now looking for someone who can help us create the forms and workflow with the aforementioned tools. However, those who are interested are most welcome to propose alternative software solutions to produce the prototype (minimal viable product).

We are LegBank and our mission is to increase access to prosthetic care in upcoming economies. We do this by supporting orthopedic labs (where prosthetics are made) to improve their day-to-day operations so they can provide high quality services for their patients. Over the last 3 months we have researched the challenges of 3 labs in Colombia. One of the key challenges is the administrative paper work they need to do, which is time consuming and papers can get lost.
We are now looking into the opportunity to turn their existing paper forms into digital forms, so they can do their administration more efficiently. Hence LegBank is searching for a way to create a prototype, making use of existing software solutions before attempting to develop software from scratch.
