Simple excel database and form producing

We are going to be attending a conference. At the conference we are going to ask people to complete a short entry form on the computer. Simple fields like name, e-mail etc.

We would like this information pulled into a spreadsheet, which is saved automatically.

When they have completed the form it should give them the line number that they are on i.e the first person that enters will be on line number 2 (assuming 1 is used for the headers) so that this number becomes the corresponding raffle ticket - which can win them a prize!

This will have to be on standard office programmes - we will not have online access and cannot guarantee that we'll have anything other than bog standard programmes on the computer.
