P&L in set format asked by a lender + Tax return for CIS subcontractor

Dear Sir/Madam,
I have been requested the following due to my LTD company wanting to raise funds via a business loan:

Profit and loss information for this financial year to date in the following format:

- Turnover
- Cost of Sales
- Admin Expenses
- Interest Paid
- Tax Paid
- Other (please specify)
- Net Profit.

My Accountant is on holiday for 10 days and I was wondering if you could help with this matter. I have all needed to be able to prepare the up to date figures.

I also have full accounts for year 2012 if you need to carry forward relevant details for this year.

I also, require a tax return for my boyfriend who worked for few months as subcontractor under CIS. If you could let me know as soon as possible I would be most grateful.
