Copy a google form and google sheet that work together

This project involves recreating a Google Forms survey and linking it to an existing response sheet to continue staff feedback evaluations that take place every 6 weeks. Previously an employee created a questionnaire in Google Forms that automatically populated responses into a linked Google Sheet. Unfortunately, access to the original Form is no longer available. However, the client has retained copies of the response sheets containing an overview of question topics and formats from past evaluations.

The goal is to develop a new Form replicating the structure and functionality of the previous iteration based on the provided materials. This will include formatting multiple question types like multiple choice, ratings scales, and short answers that correspond to the appropriate columns in the linked Sheet. Ensuring an seamless flow of submitted data from the live Form into the target Sheet is crucial.

Once completed, the new survey-response system will allow regular periodic feedback collections from staff to continue supporting workplace improvements. Recreating this workflow keeps the evaluation process steadily ongoing without disruption, maintaining consistency for analyzing trends over time. The freelancer will leverage Google Forms and Sheets capabilities to rebuild the customized solution, guided by the introductory overview and sample response data.
