Create contact database on excel spreadsheet

Using the following websites, I require a contact database created on an excel spreadsheet, divided into profession, full name, business name, email address, postal address inc postcode, website address. I am happy to pay a price per contact rather than an hourly rate and would like the list prioritised to those businesses with a website and or email address, and also a choice of costs to have just the 1) practitioner name and email address or 2) the full contact details as above.
