I need to compile contact lists of healthcare providers in 7 North Carolina towns

I need to compile contact lists of dentists in 7 North Carolina towns. The contact information must include Practice, Name, address, phone number,email (if available) and speciality (i.e. orthodontist) I need the list compiled as a cvs file. I will supply a preliminary list of some 50-60 names captured onine that need to be keystroked into cvs format. Online Yellow Page Directories are a good source.
I am compiling a complete directory of Healthcare providers in this area and this will be the first of many such assignments if completed successfully. I am uploading a sample page from the screen capture which is saved in Word .docx.
