I need some scans made into 'fillable' Adobe PDF's and my branding added

We are a new small finance company specialising in commercial finance. I have a number of items, mostly pdf files that i need to be made editable. Since i have these from other companies they are actually locked so cannot be copied. I need these files put into pdf's that i can 'fill in the boxes' i have the files and i have the branding and fonts. In total there are approximately 15 documents three of which are Word Documents and literally just need the branding put in with fillable boxes in the obvious places. Example document is below:
