Medical & Veterinary school address gathering and research

We are looking for a quick turnaround project of a list of medical schools and veterinary schools in the US and Canada, and their mailing address. The final submitted project should be a sheet containing the school name, the state/province they are in, the main contact, and two mailing addresses if possible.

The main contact we are looking for would be decision makers that fall under the assessment team, and may include titles such as:
- Program Director
- Director of Assessment
- Associate/Assistant Dean for Assessment
- Curriculum Lead

The two addresses we would be looking for would be:
- The University's Medical/Veterinary school's mailing address (not just the general school's mailing address, but that would be acceptable if the specific department's address is unrecoverable).
- The University's Assessment Office (this would be more difficult to find, and we're hoping for as many as possible).

Here are the numbers of schools we are looking for this information for, to understand the scope of the project:
- 96 Public Medical Schools in the US
- 62 Private Medical Schools in the US
- 17 Medical Schools in Canada
- 5 veterinary Schools in Canada
- 33 veterinary schools in the US

That is a total of 196 institutions to find mailing addresses for.
