I need a stylish Wordpress calculation form created

I have a Wordpress installation that needs a well built, well designed form for large format printing estimates.

Some needs are:

• No use of Flash
• Snappy in usage
• That it updates automatically when input goes in
• There is a choice of materials to print onto (e.g. PVC is one price, board is another)
• The ability to adjust prices/calculations ourselves once the form is finalised

• Contact submission email so the client gets a copy of the quote as well as us.
• Reasonable documentation for installation/adjustment

We need this to integrate into our Wordpress installation. I would be happy with a PHP script stack of some kind as long as it runs entirely internally in our site….

I have used Jotform to try what I'm doing, and a plugin called Jazzy forms almost works properly, but has stopped working, and there is NO styling info with that plugin, which is a downer.

This is a pretty urgent job.

Please supply examples of your carding/design skills also if possible. This all help your bid a great deal.
