I need a few active PDF Forms created

We have several forms, currently in Word, we need the forms redesigned and created into active pdf forms that have a submit button. The forms will have editable boxes so that when the user clicks "Submit" in adobe the form is emailed to us. Attached is a sample of another companies form that does exactly what we are trying to achieve and our form that we are trying to get to do the same.
