Hi,
I'm looking for a designer to work with me on a web app. The app is in early development stages, and I'd like to have a designer improve the look of a particular page, modal popup, and submenu in the app. The app I'm building allows people to create forms to insert into their own websites. You can see the project by by going to onform.sharkattackhq.com, click sign in, and then sign in with the following credentials:

Once logged in you'll be taken to the dashboard which currently just lists the existing forms. Click on one of these and you'll be taken to the Form Builder screen for that form, or you can click Form Builder in the side menu to see the empty form builder screen. From there you can start building a new form to get a feel for how it all works.
This Form Builder screen is what I'd like to have a designer work on. As you will see there is the page header bar, which contains the page title and two buttons, and then the content which is divided into two sections. The left column contains the form name, and draggable form components which are used to create different types of question (eg., reaction question, freeform text, email etc.) You drag these over to the builder area on the right to design the form. When you add a new question, you can click on it to bring up a modal which allows you to edit the question text and to add a reply if you want, as well as possibly other options, depending on what type of question has been added.
Also, if you click on the options button you will get a slide down set of options for the form, and a preview of the form as it will be displayed on the user's own website. (The preview only shows once a form has been saved and the app refreshed - so if you're making a form from scratch, you'll have to enter a name and save it before you can see the preview).
I'm not looking for a total redesign of this screen, instead I'd like for someone to build on what's there already; to flesh-out the current design, whilst keeping the colour scheme, typeface and general layout. However, if there are elements which you think need a full overhaul then let me know - I'm not good at visual design, so I'm open to suggestions if you think there's a need to radically change something.
For clarity, here's a list of all the elements I'd like someone to work on, all of which are within the form builder page:
- Header bar
- Form name
- Drag and drop elements list
- A draggable element during a drag (they currently go transparent when dragged)
- The form builder
- The 'drag your first question here' box
- The question and reply elements in the form builder
- The question edit modal (no need to do any designs for the colour picker at the moment).
- The options dropdown (no need to do any work on the form preview itself).
Attached are screenshots of the parts of the app I'm talking about. For now there's no need to do any work on the side menu, nor on any icons - these will come later, along with the rest of the site.
Deliverables for this project will be design files, in whatever format you like.
