Couponic Groupon clone script customisation

We have purchased, installed and initially customised a couponic Groupon clone script. We need urgent assistance to create a custom sign up form different to that given with the script, along with changing the visual design of the site, including replacing icons, and general page layout, and colouring.
We have had a developer working on this project but have parted ways as he was unable to perform in the time scale we provided.

We urgently need the work completed this week.
