Website Design/Build

I'm looking for a web designer to create a website for a new personal coaching business, focusing on health and wellbeing. My designer is creating the look and feel of the pages and I need a designer (or developer?) to build the website. Looking for 8 pages and need to include a form so users can enter their details and request and receive a short report via email (sign up box).
