Website creation

Hi,
I am looking for someone to create a 1 page sales website for one of my children’s personalised books, I have just purchased the hosting and domain name with 123-reg.co.uk
I would like the theme to be something similar to the attached image, maybe a night time scene with a sleigh flying across the top of the page and snow falling down the page? Just an idea but as long as it looks impressive I’ll go with it.
The page should have the following:
1: Paypal as payment processor
2: I would need a form for customers to fill out the personalisation details for the book and then the form would be forwarded to my email address, they could fill out the form either before payment on the page or after payment is made, whatever you think is best.
3: I would provide the text and a few images, there wouldn’t be a lot of text as the book sells itself really.
4: A “contact us” button
5: I would give you access to my hosting for you to install and test that everything works ok.
6: I don’t know if this can be done but I would like a second domain pointing to the same site if possible (already purchased)
That’ about it really for the actual page but would also like help with a few other little things regarding the setting up and forwarding emails etc.
I really don’t know how much you guys charge for this so I have just put £80 as a starter, also whoever does this site for me can then quote me for a second much bigger website for my 14 other children’s books.
