Simple website Design

I need a website created just like this one. (https://asclending.com) I can add the 800 number and logo later. The way the site functions, i need the site to do the same. So its necessary to click on all the options ( to see how things work incase the user selects any option and to function in the same manner. Its not my site but i want one just like it that can collect information in the form like the site shows ( First name, last name, phone, email, company. And the content. Its on the WP site.
