Simple competition website

I need a basic website for a trade competition website, whereby members of the public can nominate companies (from a particular sector - not ALL sectors!!) and then give them a score out of 5, for 5 different questions (e.g. customer service etc)using radio buttons or something simple. I would be happy with a wordpress website. I already have domain and hosting. I would also like to sell some adspace on the site. I would like to have this live by early November. I will provide ALL text and ALL images. I am NOT fussy about design at all - I just want it up and running asap. Probably only 5 pages. Very simple site.
