Shopify new store. I need an easy contact form on each page

Hi for a new shopify store I need a contact form on every product page so the customer can ask a question. Very simple form they ask the question and enter their email. When the form comes to me I need to know what product page it came from. Or another solution if you can suggest. Need to interact with customers. Also need a bit of general advice to make sure the store is set up well. Can split any work up into separate jobs if needed.
