Redesign existing Wordpress Website -- PHP, CSS, JS skills required!

Hi,

I need to redesign a wordpress website. I know myself how to install plugins, change the design, add elements and all basic stuff. What I need is customization (example: this mobile menu is installed: http://preview.codecanyon.net/item/taptap-a-super-customizable-wordpress-mobile-menu/full_screen_preview/9966828 --> "demo two" with the nike show. I need to replace the "find" feature with a clickable telephone number. Nothing too complicated but you need to customize the code) and someone who can add features that I need and which aren't implemented by default in the theme.

I also need someone with a good design / taste who will not just change a couple of colors of a default theme but will make the website look unique and professional. SEO (Yoast) optimization is also needed.

Please provide me with examples of websites that you have designed and how good you're with PHP/CSS/JavaScript.
