Online Questionnaire with results show as a Graph update!

Summary

Current status
• We currently have a 35 question questionnaire is on a wordpress website
• On completion of the questionnaire an email is generated to our admin department where it is manually calculated

Desired status
• Fully automated system
• Questionnaire completed, the person who completed the questionnaire is emailed the results, with the offer of a coaching call to debrief them (for a fee)
• Results from questionnaire are added to a database
• Email address of person who completed questionnaire added to customer database
