Extra notes: I'm looking for a designer/marketer to design and develop a professional high converting attractive fully responsive landing page for my Personal Training Business. Need it to be similar or better than ( http://markeazy.com/preview?theme=Top-Trainer ) using the content (logo, bio picture, testimonial, copy) and overall color cheme of my Personal Trainer website ( www.procertifiedtrainer.com ) and built via HTML/Wordpress content management system.

I will want the lead capture sign up form to be slightly different with a field that collects (Name, Email, Phone Number, Whats Your Specific Goal?) like the one at the end of my website www.procertifiedtrainer.com and some other elements changed but not much. Also, I would also like the contact form to send the submissions to my email address.

You can forward me any examples/samples of your capabilities, but not necessary.

* Must be able to set up & install on my Hosting account Godaddy.com
* Intergrate Facebook page platform.


NOTE: Chosen Designer who does a phenominal job with this particular assigment will have the opportunity to work with me on a long-term basis performing same task for my (PT Business Consulting Practice) creating lead generation landing pages for other Personal Trainers.
