want to have a page/template designed for my shopify site www.dungeon-eyeswideshut.co.uk the page layout the exact copy in the colours/style of the dungeon site. the functions the same.

The page/form should be able to be sent to the mistress for them to fill out and email back for admin to approve.

you will not be work on the live site it will be developed on a copy of the site and once i have check all is ok and then it will be upload to the live site.
