To create an interactive assessment form in adobe. The assessment form has two parts. Section one has 9 questions which should be straightforward. Section 2 has 3 parts where clients are required to choose specific products each individually priced. The selection of the items should be reflected in the total price. The form once completed needs to be sent to a different organisation. I'm wondering if the designer can come with an idea to enable us to only email section one and hide section 2. I would like to have both sections in one lot to make it easier for customers and also admin purpose.
Also there is one question in the section one which asks client if they are tax exempt. The answer to this question needs to be reflected in final price (@20%VAT).
The form will be emailed to the successful bidder.
