Wooden Candle Box

I am looking for Wooden Box for our Candle jar. We already have some idea which is attached to this project. You must provide following:

1. At least three different boxes (2 large and 1 small box
2 Each box should have inside Ethylene Vinyl Acetate (EVA) design, this will show our 3 jar for 2 large box and 1 jar for one small box
3. We need accurate 3d and 1d design for production with full measurements.
4. 3d render to show what box should look like
5. All necessary file must be provided for final selected box such as .ai, .pdf and other graphic required format.
6. Must be able to complete the project on time and efficiently.
7. Must be able to communicate quickly and daily.

We also required full merchandise design so wining bid also will have opportunity to work with other designs as well.

We already have logo and required text to go on box inside and out side
