Word document watermark. I need to replicate something.

I need a 8.5"x11" watermark design to be placed in a MS Word document. Its actually two labels. It needs to exactly match the Example below.

1) The City seals shall be in a half-tone.
2) The words Time:, Date:, TMP#: and Comments shall not be half-tone (full black).
3) All proportions must remain the exact same as the example.
4) The fonts must match.
5) Lastly, please advise me what font the times, dates, OCCU# and address are written in.
6) I need this ASAP.

Basically when I place the watermark in the Word doc, I can then type the times, dates, OCCU# and address - then it will match the Example.
