Web site alert box elements

Hi I am looking for a graphic designer for a simple job.

Basically I need alert box web elements. There will be 6 types boxes.
Success
Warning
Error
Info
Note
Normal

These boxes will have related icons except the normal box(This will be default box type). The design should be clean, user friendly and professional looking and most importantly css friendly from photoshop format. At the end of the work, if the both sides are happy, there will be following projects.
