Video Game Poster Art Illustration

I have an idea for an image to use as an advertising poster for a video game I am making.

Here's a summary of the game to give you a feel of what I need in this poster.

It's a bizarre shooter game called M.E.A.T Nurse. You play nurse who collects blood for use in surgeries using a special blood collecting shotgun.

I've created a rough draft of the image that I want you to use as inspiration and I've written a detailed description of my idea as well.

The poster shows the MEAT Nurse in a locker room in front of his open locker.

He is wearing blue nurse scrubs trousers and cap that are both heavily bloodstained. He is bald. He has a muscular back.

He has back tattoos of the Caduceuos staff and DO NO HARM with the no covered by tattoos of shotgun shells.

He holds a large scalpel in his left hand and a sawn-off double barrell shotgun in his right resting on his right shoulder

There are 3 elements to the back tattoo
1. The Caduceus seprent staff

2. DO NO HARM written in an arc at the top of the back.
3. The NO in DO NO HARM is crossed out with some shotgun shells.

I don't know exactly what I want each element to look like so please do some research and experiment and see what inspires you.

In his locker there is a top shelf with a book lying down called M.E.A.T Bible. On top is an employee of the month picture of the nurses face wearing his cap, welding goggles and facemask.

Underneath the shelf is a clothes rack with bloody scrubs hanging on it.

On the inside of the door is a recruitment poster that is inspired by the uncle sam WW2 posters. It says M.E.A.T needs you and You blood is our business. Uncle sam should be wearing a face mask.
