Multimedia Designer/Artist For Music Project/CD/Artist Launch

Multimedia Designer/Artist = Graphic, Web, Illustrator, Typography, Photographer, Animator, Film/Music Video Maker etc.

My name is Shak, and I would like to launch my debut EP (mini album) as an independent singer songwriter, using the help of a designer/artist who has worked with musicians before.

I would like to create a visual identity for myself and my music.

What I need:

*One visual/concept that I could use across all online/offline platforms (CD, social media, website, flyers, T-shirts etc.).
*Shak logo (I'm not a company, so it's not necessarily going to be something I'll be using for years to come).
*Basic website (no programming – using one of the free platforms)
*Music video (Animated or “Normal” or a mixture of both)
*Album/publicity shots

If you're just a photographer or just do video animation or create music videos that's fine.
However, for the design part, I'm looking to work with someone who's both a Graphic designer, has enough web knowledge and experience to help me create my basic website and perhaps other web pages (Facebook etc.), and preferably is also an illustrator and/or a typographer, although not a must.
The web part is secondary so I want someone who's first and foremost an ARTIST in the old school sense, and does stuff with their hands too :)

Since it's only my first release, there's no point in splashing out, and I'm not looking to create anything grand.
As for the EP, aside from the visual/s, I will just have credits and maybe song lyrics, although song lyrics are not a must, especially if they make it more complex/expensive.
I need something fairly minimal that communicates coherently and stands out.
Once the design is ready, I will use it to create my facebook and other online pages, as well as a website using a free platform such as https://www.fourfour.com or http://www.wix.com/ and as I mentioned, I would need your help setting up the website.

Whether it's logos/other designs, I generally prefer visuals that are less cluttered and don't look like someone was trying too hard to be clever/impressive.
Depending on the project/style of music, I feel simplicity generally creates more focus and impact, and I prefer visuals with few colors/shapes.
I am not drawn to fancy 3D stuff and amazing technicolor, and prefer things more raw/unpolished but still with edge, impact and style.

The EP will be named "Bird On The Staircase", after one of the songs.
As for the design, I have an idea that involves typography (showing my name, and having the "S" look like a spiral staircase, while making the "k" look like a bird/wings as it already kind of is). However, I'm not locked on it and I'm open to using photos, illustration, collage etc. and any other suggestions you might have.

Don't pay too much attention to the design of it - it's something I chose quickly from a bunch of ready made myspace themes.

If you're fairly inexperienced that's OK, as long as you're HIGHLY/UNIQUELY talented.
