I need a book cover that looks a bit like a painting and has a satirical edge.

Information for the business: I have a screen novella that needs a book cover.
Design details: The book cover will have a painted quality and be somewhat satirical (the book is political satire) - NOT cartoonish. It will feature:
1. A queen's crown (in gold or silver). It should have 50 diamond stars (not too obvious, so they can be small or each could consist of tiny diamonds in a star shape), and one larger star in any jewel.
2. Coming out of the top of the crown will be rabbit ears and three slender missiles.
3. Attached to the edge of the crown will be American and Union Jack flags intertwined (not shown in the attached sample).
4. Somewhere below where the mouth might be would be very prissy screwed up lips (a bit like an oval or "O") of a woman in her late 60s. In the sample, the lips are pretty good, but they need a few wrinkles added.
5. The final element would be either a red heart (it could optionally be faded) somewhere in the background. In the sample, it's pink, but I'm not so certain I like that, and it doesn't have to be directly behind the crown/lips. Optionally, you could show one or all of the cards for a Royal Flush in the Heart suit (cards 10, Jack, Queen, King, Ace); you could place them wherever to balance the image. If you use just one, it would be the Queen of Hearts.
6. The background shouldn't distract from the remaining elements. So whatever you think would work.
7. The book title now has a subtitle (The Queen's Plan), so you'd need to make room for it below the title).
The book is somewhat "intellectual" if that helps you ponder the design.
Target audience: 30s-100s
Ideas for the visual style: See attached sample. It's not precisely what I'm look for, but it's along the right lines.
Things to avoid: Cartoonish look. It should look more realistic, but let potential readers know that the book content is fun satire.
Preferred colors: Crown should be gold or silver. If jeweled, the jewels should be mostly diamonds so as not to put too much focus on the crown's colors.
Where the design will be used: It will be used on a Kindle book and possibly a print edition.
