Artwork for a transfer tattoo from an existing logo

I need two logos turning into artwork (end result needs to be similar to attached eproof)
Both logos will need resizing and one will need minor adjustments to simplify design.
Page two (reverse) of artwork will be same as on attached with minor alteration to text on each.
Please note, I will have similar small projects in the future as well so would like to find someone who I will be able to use in the future as well.
I would need this completed by 16th April at the very latest. I would like the artwork in pdf format and either ai or eps format.
