WORD to PDF then making PDF editable boxes (text and tick box)

We have a number of documents to make PDF -editable.

I have attached the first one. needs converting to PDF and all the answers need either a editable text box or tick box adding. If you could provide a sample of two pages please.

We have additional documents that need similar work. These vary in size and requirements (to make pdf-editable). Please can you therefore provide a per hour rate for doing the remaining forms.
