Description
* Current mnemonics need to have LX in front of them e.g. PISNA--> LXPISNA etc
* Eight more consultants need to be added, along with their mnemonics.
* Amend 3 consultants from Miss to Ms.
* Add Thrombophilia screen 2 to the heamatology section.
* Add the following to the Additional Services section:
EMMA and ALICE
Pessary
* Version control: add V15 Jan 2026 to the bottom right corner of one page.
* Amend typos in the lab addresses. Change to:

Once those updates have been made, a fourth form needs to be created for a new site. It will be a duplicate, but with a different address.

Good day, London. What is the deadline for submitting your project? How many documents would be included in the project?

Will you be providing the original editable files (Word/InDesign), or should the edits be made directly in the PDFs?
Do the eight new consultants already have confirmed mnemonics, or should I format them following the same naming convention?
For the three consultants changing from Miss to Ms, can you confirm their names so I update the correct entries?
Should “Thrombophilia screen 2” follow the same formatting and ordering as the existing haematology items?
For the fourth form with a new site, is the layout identical with only the address changing, or are there any other site-specific details to adjust?
Do you want the version label “V15 Jan 2026” applied only to one specific page as mentioned, or consistently across all pages?
Is there a preferred way you’d like the updated files delivered and named to match your document control system?
