Update existing PDF form.

Information for the business: Community Services
Design details: I would like you to update fields and make minimal changes to design. Colourwise leaving form good for B/W printing.
Target audience: Public and other organisations
Ideas for the visual style: use existing design or suggest changes
Where the design will be used: print, web, emails.
Extra notes: I need update existing PDF form.
Current PDF form hardly can be called PORTABLE as it is over 6MB while being only 3 pages.
Not good for storing or passing data via email.


All field require checking for compatibility as next step plan to use this form for DATA Export/IMPORT to and from EXCEL.
