SIMPLE APP DESIGN FOR RECORDS MANAGEMENT APP

We require this application designed
for apple and andriod
the design is to be clean and can use any template
as long as the design is upto date
To use hues of blues and whites some green and a little red can also be used

We have 8 main screen with some that have a small variance of that screen (eg personal info screen and edit and save personal info screens)

We have a total of 13 screens to be designed.
