Roller Banner design required

I require someone to design a roller banner (see measurements attached) which they can send me the file for so I can go ahead and print it on vistaprint. The roller banner is to promote an independent opticians & to promote having regular eye examinations.
I will provide what content I would like on the banner however I require someone who has access to imaging websites like shuttershock.
I need this ASAP!
